Prior to the Community Animal Centre Manager or their approved designate completing this form:

The staff member must have completed a Pre-Adoption Questionnaire, which has already been reviewed by the Centre Manager where the animal is located.
The manager at the location where the animal is located will have determined if the animal is a suitable match, and determined priority sequence in the queue among public interest (public matching applications take priority if received first; staff don’t get priority over public).
If the staff member is determined to be the first suitable match in the requesting sequence, their standard Pet Adoption Application must be uploaded to the document manager of that animal’s ShelterBuddy ID prior to this online form being completed by the Centre Manager.
Once the staff member has received conditional approval from the manager of the centre the animal is located at, and their application has been uploaded to ShelterBuddy, the Centre Manager or their approved designate can submit the form below.
